I have access to a 30.06 barrel that may have a .308 insert/adapter stuffed into it. Is this thing safe to use? Was this a common fix for problem throats or just a cheap conversion that is best left alone?

Let's look at the worst case scenario, shall we? Let's say your cursing along shooting your .308 and SUDDENLY, the insert falls out. Now your shooting .308 out of a 30/06 barrel, DRATS! Has anyone ever shot .308 out of a 30/06 barrel? I have, Punch the Clown has, the rounds go off just like they were 30/06 and they even are pretty accurate with one little change... The brass now looks like a 45/70 when it's spit out :)
How excessive is the headspace when you shoot .308 out of a 30/06? It's got to be pretty severe doesn't it? If the brass is in good condition though I've not seen even one incident of bad juju.
In my opinion, I'd go for the insert. There's even a company in Alaska that makes those .308 inserts for use in any of your 30/06 barrels. So, with no fuss no muss you can turn your 30/06 into a .308 instantly.

I have no personal experience with these barrels, but I read somewhere that these had the chamber faces reamed out to accept the inserts so I don't know if that makes a difference, but I'd use one.

Thanks again for the input. I will take a 'shot' and get the barrel for another project. ZB.

Not the first one i have seen posted..I wonder if the insert stays on a ejected round or falls loose after firing?

You should be able to see the insert if the barrel isn't mounted...I always wondered just how they scored or reamed or modifyed the barrel.

I remember reading somewhere that the chamber was scored then the bushing was pressed in and snapped into the score and was supposed to be held in place

Okay, good info, I own a bushing from years ago, and the directions with it are sketchy..I was pondering the use of permanent type Threadlock to secure the bushing, if i used it in my garand..Hold it in place with a fired casing for about 20 minutes..BTW the paperwork said it was Navy surplus..just steel ring in shape to fit cartridge shoulder..
